Safe Medical Devices Act of 1990 If a piece of Equipment is involved in an incident resulting in the death or injury of a patient, it must be reported! • Report the occurrence to your supervisor

• Take the equipment out of service • Mark the equipment as defective • Sequester the equipment until it is picked up by Bio-Med

• Complete an Occurrence Report and reference the equipment “C” number Broken, Defective Equipment & Hazards in the workplace? Who do you call?
