Care Coordination The Care Coordination team is a dedicated group of healthcare professionals that work with patients to ensure that the highest level of integrated and personalized care is rendered while ensuring that services are delivered in an effective and timely manner. Members of the Care Coordination team work collaboratively with the patient, their families and third party entities to facilitate communication and ensure all linkages and arrangements necessary to help an individual transition effectively from the hospital to the most appropriate next level of care is achieved.
